Sound Body Sound Mind Surveys

The completion of these surveys is required of all schools who have received the Sound Body Sound Mind Fitness Center Grant in 2012 and later. Surveys must be completed directly before and after instruction of the Sound Body Sound Mind Curriculum. The Sound Body Sound Mind Foundation requires each school to instruct the Sound Body Sound Mind Curriculum once a school year for 5 consecutive years following installation of the Sound Body Sound Mind Fitness Center. Failure to complete these surveys will result in ineligibility for further funding opportunities from the Sound Body Sound MInd Foundation such as the Fitness Center Maintenance Grant and Afterschool Support Grant as well as possible removal of donated equipment.
